Post-Treatment & Progress Follow-up

Post-Treatment & Progress Follow-up is a dedicated telehealth visit to make sure your care is actually working for you-not just written on a chart. Whether you were recently treated for an infection, started a new medication, adjusted your blood pressure regimen, or began a skin or weight-loss plan, this appointment focuses on how you are truly feeling now.

We begin by reviewing what has changed since your last visit: symptom relief, new concerns, side effects, sleep, mood, and how well the plan has fit into your daily routine. If you had a procedure elsewhere or visited urgent care, we go over discharge instructions and any test results you can share so you are not left trying to interpret them alone. From there, we combine two focused follow-up angles. First, we use a Medication Management Follow-up lens to examine how each prescription is performing. We talk through missed doses, side effects, costs, and practical barriers, then decide together whether to continue, adjust, or switch therapies. Second, we provide a Symptom Progress Evaluation, looking closely at what has improved, what has plateaued, and what might signal the need for further testing or in-person care.

Because you always meet with the same clinician, we remember the details of your history, not just the last note in your chart. That allows us to fine-tune your plan in a way that respects your preferences, schedule, and other conditions. At the end of each follow-up, you leave with a clear next step: updated prescriptions when appropriate, symptom goals to watch for, and a timeframe for your next check-in.

This visit is meant to give you reassurance and direction after treatment, so you never feel like you were given a plan and then left to figure out the rest on your own.
